Three questions we start with
Which device? A Bard PowerPort or related Bard port catheter. If you are not sure, that is fine — a participating firm can pull implant records.
What happened? Fracture, migration, perforation, blood clot, infection, or removal surgery. Medical treatment for the complication is the key marker.
When and where? The implant date, the complication date, and your state. These determine which deadlines apply.
